RehabilitationJob Summary
We are searching for a Rehab Care Coordinator (Registered Nurse) -
someone to serve as an integral member of the interdisciplinary
Inpatient Rehabilitation Team through proactive planning,
organizing, coordinating, monitoring, and evaluating services
provided to patients admitted to the IRU. Responsible for ensuring
consistency and quality of the referral/admission process, direct
patient/family pre-admission assessment, coordination of patient
admission to the IRU, team collaboration to ensure a safe
discharge, referral source marketing and community liaison.
Responsible for ensuring payer funding through verification and
securing of admission pre-authorizations, continual case management
of patient goals and outcomes within effective and appropriate time
frames and ensuring that payer/regulatory conditions are met.

Responsibilities
* Conducts a thorough and systematic pre-admission assessment of
the patient's current status, addressing the following components:
Current and past medical history, physical/functional/cognitive
level, psychosocial/ behavioral issues, environmental/ cultural
needs, support system involvement, and financial resources
* Coordinates, facilitates, monitors, and assesses the plan of care
for patients throughout the episode and continuum of care to
achieve the optimal clinical and financial outcomes. Oversees
discharge planning and facilitates safe transitions to community
settings
* In collaboration with the Care Management Department, serves as a
liaison between members of the interdisciplinary care team,
community providers, payer sources, and patient/family to ensure
safe and effective plans and smooth transitions between internal
and external levels of care
* In collaboration with the Care Management Department, effectively
and efficiently oversees the plan of care to optimally assure
clinical and financial outcomes
* In collaboration with the IRU interdisciplinary team, identifies
opportunities for improvement, participates in data collection and
analysis, contributes to problem solving and actions to be taken
and assesses effectiveness of those actions implemented
* Within the nursing scope of practice, the IRU Care Coordinator
continuously assesses self-knowledge and competencies to ensure job
performance
Qualifications
Being fully vaccinated against COVID-19, including any booster
dose(s) of the COVID-19 vaccine recommended by the Centers for
Disease Control when eligible, is required for all employees at
Texas Children's, unless approved for a medical or religious
exemption
* Bachelor's Degree In Nursing OR Associates degree in Nursing or a
related field AND current enrollment in a BSN program and signed
agreement, OR must be currently enrolled in a nursing bridge
program working toward an MSN. Verified BSN-equivalency or a signed
agreement is required
* RN - Registered Nurses Texas Board of Nursing or Nursing
Licensure Compact
* BLS Required
* 3 years Professional nursing experience required
* Experience in Case Management/Coordination, Quality Management,
Utilization Review, or discharge planning preferred
* Experience in Pediatrics and/or CRRN (Certified Rehabilitation
Registered Nurse) preferred
* Bilingual preferred
