Pearland Medical Center specialties: Our Women's Diagnostic Imaging Center offers the most comprehensive care in the Pearland area, using the very latest in imaging. Our 3D mammography is the first of its kind in the area. It offers the most accurate imaging of any technology ever developed to detect breast cancer early. Pearland Medical Center's Cardiovascular Services will feature a cross-functional team of cardiologists, including the first stenting services offered in Pearland. Our Imaging Center will offer everything from X-rays to CT scans and more. Plus, we have experienced radiologists to analyze every detail, ensuring you have the absolute most accurate diagnoses. Our Outpatient Services have everything you need in one place: imaging, diagnosis and treatment. Surgery in Pearland from world-class surgeons? Of course. General surgery, spine, GYN, urology and much, much more. Pearland Medical Center is currently accepting applications for Registered Nurse (RN) Case Manager - Case Management . Position Summary: Responsible for facilitating the progression of the patient through the continuum of care. Actively participates in the collaborative program which requires review of patient activities related to medical necessity of hospitalization, appropriateness of length of stay, timely and appropriate utilization of services rendered, and identification of patient needs related to patient care following discharge from the hospital and evaluates the plan to determine any modifications or changes needed. The Case Manager promotes quality care and cost-effective outcomes to enhance the physical, psychosocial and vocational health of individuals. The position encompasses reviewing, planning, implementing, coordinating and evaluating health-related service options throughout the continuum. The employee will be responsible for knowledge and skill necessary to provide care based on physical, psychological, educational, safety and related criteria for the neonate, infant, child, adolescent, adult and geriatric population. Responsibilities: 1. All employees will be held accountable for attending training regarding the Code of Conduct program at least annually 2. Appropriately utilizes all resources and available sources of information. 3. Applies problem-solving techniques to the Case Management process 4. Communicates with outside review agencies as necessary, and maintains appropriate professional contacts and liaisons. 5. Documents medical necessity review for established level of care in the MIDAS utilization review module. 6. Documents all Case Management activities including Discharge Planning 6. ESSENTIAL DUTIES AND RESPONSIBILITIES: Evaluates assigned population of patients for risks and potential discharge needs. All patients will be assessed for Discharge Planning upon admission by a Registered Nurse (R.N.) as part of the nursing assessment. If appropriate, the R.N. will generate a referral to Case Management, based upon high risk, problem prone diagnosis and/or psychosocial needs. The Case Manager or Social Worker will assume responsibility for completing a Discharge Planning Assessment on the identified high risk, problem prone patients referred to the department. 7. Establishes and coordinates plans for those patients with identified needs. 8. Ethics and Compliance Expectations - All employees will be held accountable for behaviors displayed relative to the values articulated in the Mission and Values Statement. The Case Management Department will adhere to and comply with HCA's ethics and compliance policies and procedures. 9. Maintains current knowledge of regulatory rules and guidelines, as well as guidelines for contracted insurances. Remains knowledgeable regarding all applicable standards, guidelines, limitations, and restrictions. 10. Monitors, evaluates, and documents care outcomes using CMS or Inter-Qual guidelines. 11. Participates in committees or departmental activities as might be assigned. 12. Refers appropriate cases to management and social worker as needed. 13. Collaborates and works with patients, families, Case Management staff, physicians, nurses, and other clinical disciplines and healthcare team members in making decisions regarding social and discharge needs to facilitate efficient, appropriate care through coordinating case management activities for patients. Qualifications Qualifications: RN licensure by Texas Board of Nursing Minimum for 3 years of clinical practice with emphasis on leadership and management exp, preferred Previous experience in critical care nursing with certification (CCRN), preferred BLS and ACLS from the American Heart Association, required Current licensure in Texas as a Registered Nurse with Bachelor of Science in Nursing, preferred Experience in health care delivery systems with focus on Case Management or Performance Improvement, preferred
