Job Description:

*Job Summary: *

Works closely with primary care teams to provide short term care coordination and connection to resources to clients assigned to the program. Works in both clinical and community-based settings.

*Summary Specific to Job: *

- Abide by all the duties and responsibilities of a Community Health Worker according to state regulations

- Serve as a liaison between clients, hospital centers and community organizations. Assists clients in accessing health related services, including referrals to in house case management services

- Provides support and advocacy during initial client referral by conducting a warn hand off to in house case manager

- Coordinate client transportation and communication between providers and serve as a liaison between clients, health care providers and community organizations

- Documents all client encounters, completes and submits weekly monthly reports; maintains comprehensive electronic client files, which include client notes, release of information, assessments and other medical documents.

- Coaches clients in effective management of their resources. Assists clients in understanding care plans.

- Maintains a polite and professional attitude including communication via phone, verbal, e-mail with peers, clients, community, and all involve in the project

- Proactively identifies and reports and resolves problems.

- Adheres to agency policy, procedures, and the professional code of ethics.

- Attend all meetings and trainings as requested

- Adapt to changing work requirements along with flexibility of time

- Perform coaching assignments onsite and offsite to include any additional duties assigned

*Minimum Requirements: *

- High School Diploma or GED

- Community Health Worker Certification required

- Have extensive knowledge of community agencies

- 46 Hours of Recovery Support Specialist Training and Certification preferred

- Driver's License and good driving record

- Experience with Microsoft Office Suite

*Ability to: *

- Ability to communicate well oral and written/ Bilingual preferred

- Ability to communicate medical information to health care professionals and care coordinators over the telephone

- Ability to commute within assigned precincts in Harris County

- Skill in use of personal computers and related software applications, including e-mail.

- Skill in organizing tasks and establishing priorities

- Creative and analytical thinking

- Have extensive knowledge of community agencies and their services

- Work cooperatively as a team member

- Accept and follow directions that comply with organization guidelines

- Work cooperatively as a team member and independently

- Multi-tasking skills is a must

Job Type: Full-time

Pay: $13.17 - $22.00 per hour
