today! Responsibilities RN Case Manager Responsibilities
- Assess all medicines to identify possible ineffective drug
therapy or adverse reactions, significant side effects, drug
allergies, and contraindicated medications.
- Establish realistic measurable, observable goals consistent
with patient's diagnosis.
- Comply with legal requirements, standards, policies, and
procedures including, but not limited to Compliance Process: Code
of Conduct, HIPAA, and documentation standards.
- Demonstrate professional conduct, service excellence and

Responsibilities
- Provide nursing visits in accordance with the written plan of
treatment signed by the physician and all subsequent orders that
authorize and direct changes.
- Provide paperwork as required by the agency to maintain
accurate and complete medical records.
- Conduct and pursues the business of the agency in accordance
with sound nursing and medical practices and represent the agency
in a manner calculated to instill confidence in patients,
physicians, and the general public.
- Perform nursing services only under the guidance and
supervision of a registered nurse.PT Responsibilities
- Assess and evaluate therapeutic/rehabilitative/functional
status and participate in developing the total plan of care.--
- Direct physical therapy treatment.--
- Instruct patients, families and caregivers in the use and care
of therapeutic appliances.--
- Prepare and submit clinical notes in a timely manner.--OT
Responsibilities
- Assist the physician in evaluating the patient's level of
function by administering and interpreting patients' diagnostic and
prognostic tests of function in their homes
- Participate in the development and revision of the Plan of Care
by conferring with the patients' physicians and clinical team
members
- Improve and restore strength, coordination, range of motion,
and function by providing therapeutic treatment and instruction to
patients in accordance with physician orders. QualificationsRN
Qualifications
- Graduate from an approved school of professional nursing and
currently licensed to practice as a registered nurse in the state
of agency operation.
- One year experience as a Registered Nurse
- Must possess and maintain valid CPR certification while
employed in a clinical role.
- Has sufficient endurance to perform tasks over long periods of
work hours.
- Must be a licensed driver with the ability to travel to all
business locations.LVN Qualifications
- Graduate of accredited practical/vocational nursing
program.
- Currently Licensed Practical/Vocational Nurse (LPN/LVN) in the
state(s) of agency operated.
- Must possess and maintain valid CPR certification while
employed in a clinical role.
- One year of clinical experience and one year of home health
experience.
- Must be a licensed driver with an automobile in good working
order.PT Qualifications
- Master's degree in physical therapy approved by CAPTE
accredited physical therapist education program.--
- Must be currently licensed to practice as a Physical Therapist
within the state of agency operation and compliant with accepted
professional standards and practices.--
- Minimum Nine (9) months appropriate experience as a physical
therapist.--
- Community/home health experience is preferred.--
- Must possess and maintain valid CPR certification while
employed in a clinical role.--
- Valid driver's license and an automobile that is insured in
accordance with state and/or home health requirements and is in
good working order.--OT Qualifications
- Master's degree in Occupational Therapy or the equivalent with
license/certificate within the state providing services
- Knowledge of rehabilitation techniques related to complex
neurologic injury generally required
- Experience with home health, medical-surgical, or
rehabilitation center environment
- At least two (2) years of experience working independently
under minimal supervision
- Must be a licensed driver with an automobile that is insured
and in accordance with state and/or organization requirements and
