Responsibilities:
RN / Registered Nurse Team Director Responsibilities:
Personnel Support / Management: Supervise direct patient and family
care team members, in collaboration with program site leadership.
Responsible for scheduling, payroll and other administrative
duties. Ensure all initial assessments and required reassessments
are conducted by qualified team members within required time
frames, including documentation completion, submission and/or
synchronization. Ensure the 24-hour availability of nursing and
psychosocial services to patients and families and coordinates
"on-call" assignments. Oversee and support the Interdisciplinary
Group (IDG) in development, implementation, and review of the
interdisciplinary plan of care for each patient and family. Ensure
all patient and family services comply with federal, state(s) and
local regulatory requirements and with accepted professional
standards. Collaborate with program site leadership members to
monitor, manage, and mentor staff in patient care activities,
productivity, and documentation per related policies and protocols.
Participate in the selection and evaluation of all direct patient
and family care team members
Education: Participates and promote Back to Bedside efforts in
reinforcing Case Management skills, knowledge, and attitude per
protocol for all disciplines. Provides external education and
in-services to community partners. Collaborate with program site
leadership to support staff education efforts. Participate in
orientation and ongoing education of staff and volunteers
Interdisciplinary Group: IDG Meeting Preparation: Prepares patient
and care quality data analysis (may include Adverse Events, triage
trending, med costs, red/yellow patients, OA, Advance Directive
issues etc.) to provide timely staff support and feedback as
necessary during and/or after meeting. May review above prepared
data analysis with Associate Team Director (ATD) prior to IDG
Meeting and coordinate staff support strategies. Facilitates the
IDG team meetings: Engages and encourages all IDG participation.
Prepares and follows up on identified patient care concerns.
Coordinates with ATD/ Team Assistant (TA) to ensure regulatory
documentation requirements addressed as needed. Ongoing IDG
Support: Utilizes proactive management strategies to assist staff,
patients and their families, which may include: Triages incoming
calls during business hours, Provides caller support and
assistance, Performs quality calls.
Investigate (root cause analysis) and follows up with staff to
process issues in collaboration with ATD and other program site
leadership as needed, and documents outcomes per protocol
regarding: Adverse Events, Triage Trending results, Quality of Care
Calls, Circle of Care Calls, Deyta / CAHPs and other quality
measures / outcomes. Participate in or leads quality improvement
activities, including data collection and aggregation as able.
Participate in ongoing quality assurance activities related to
patient and family services, including Quality Assurance &
Performance Improvement (QAPI) studies and chart audits to ensure
the quality of patient and family services
Communications: Reinforces and supports all internal and external
communications related to: Interdepartmental information,
administrative process changes and messaging, participate in the
development, implementation and evaluation of policies and
procedures for patient and family services, Ensure all
interdepartmental communication and exchange of data is accurate,
timely and effective, Provide proactive customer service
maintenance. Communicate as needed with members of the team, the
patient and their families, the Medical Director, the
Director-Clinical Operations, and others involved in the care of
the patient to facilitate optimal care, especially during
transition situations
Qualifications:
RN / Registered Nurse Team Director Qualifications:
Associate degree, preferred; Registered Nurse license in state(s)
practicing, required
Certified Hospice Nurse (CHPN), preferred
Current Cardiopulmonary Resuscitation (CPR) certification,
preferred
3+ years' experience in hospice, home health, general medical
surgery, oncology or patient population specific (i.e.: geriatrics,
pediatric, etc.) nursing
Minimum of one year of supervisory experience
Must have reliable automobile, valid driver's license, and the
