Legacy Community Health -is a premium, Federally Qualified
Health Center (FQHC) that provides comprehensive care to community
members regardless of their ability to pay. Our goal is to treat
the entire patient while improving their overall wellness and
quality of life, in addition to providing free pregnancy tests,
HIV/AIDS screening. At Legacy, we empower patients to lead better
lives by promoting healthy behaviors and offering resources such as
literacy classes, family planning services, and nutrition and
weight management information.
Our roots began in 1981 as the Montrose Clinic, with specialization
in HIV education, testing, and treatment. Since then, the agency
has expanded to > 50 clinics in Houston, one in Baytown, 3 in
Beaumont, and one in Deer Park with extensive services that
include: Adult primary care, HIV/AIDS care, pediatrics, OB/GYN and
maternity, dental, vision and behavioral health. We also service
students within KIPP and YES Prep schools. Legacy is committed to
driving healthy change in our communities.Job DescriptionThe
Medical Director for Adult Medicine Services is a board certified
physician in the area of his/her field of expertise. S/he defines,
promotes, and reviews quality practices for his/her department and
the sites in which providers work. S/he serves as a community
representative in his/her discipline. S/he develops processes for
ensuring continuity across patient transitions of care. She/he
works towards implementing initiatives for improved health
outcomes, clinical efficiency, and patient experience.Essential
Functions:
- Directly oversee the growth and sustainability of the Adult
Medicine Service Line, including specialty and primary care adult
services.
- Works intricately with the medical directors of other service
lines to ensure alignment across the services at Legacy,
- Educate site medical directors on expectations of providers in
your discipline; monitor and support providers to improve in their
clinical practice
- Set standards of care/protocols on the management of common
conditions
- Provide evidence-based guidelines and decision support tools to
aid providers in care
- Define productivity standards in your discipline; Act as a
resource to help guide Site Medical Directors on the productivity
expectations for their providers in their respective service
lines
- Recommend and create implementation plans with key stakeholders
for pilot projects or change initiatives
- Define and oversee panel size and distribution for providers
based on their level of license
- Serve as a community representative for Legacy in
activities/collaboratives/committees related to your
discipline
- Represent your discipline in the budget process
- Recruit providers and oversee onboarding into your discipline
for the organization
- Oversee monthly service line meeting for your discipline
- Investigate and propose pilot testing of new initiatives and
models of care
- Oversee quarterly Peer Chart Review process, Ongoing Monitoring
and Performance Improvement Plans
- Ensure ongoing educational activities support providers
up-to-date care practices
- Collaborations with academicians in your specialty
- Manages and designates On Call Schedule for emergency calls
after hours
- Develop strategies that manage transitions of care for our
patients
- Serve as link between EHRS/IT team at the local level and with
our vendors to ensure that discipline related content is up to date
and relevant and that providers use of the system is optimized
- Serve as link with discipline specific educational and training
programs to establish and strengthen opportunities for teaching at
our sites within our department
- Advise Site Medical Directors and operational site directors on
implementing improvement strategies in discipline specific
processes
- Work with Grants team to identify opportunities and assist with
implementation of new projects
- Work with Billing department on best practices to optimize
compliance and revenue in the department
- Advise on major medical equipment purchases related to
department
- Work with Marketing and Community relations team to ensure
discipline specific outreach is done to increase visibility in
community, increase patient traffic as well as promote clinically
relevant messages
- Work with other service line directors to establish and support
a collaborative interdisciplinary model of care
- Prepare department for clinical audits and ensure that clinical
practice is aligned with audit requirements
- Work with legacy leadership to identify, train and update
providers in value-based care.
- Work on research projects in collaboration with the research
team/pharmaceuticals.
- Maintain regular clinic hours for a percentage of FTE as
determined with CMO and may fluctuate with the need of the service
line
- Performs other duties as assigned. Knowledge, Skills, and/or
Abilities:
- Physician with at least 7years of clinical practice experience
post-training
- At least 5 years of leadership/management experience
- Credentialed and privileged according to Legacy's standards and
practices
- Must be able to manage multiple deadlines and prioritize
- Demonstrate skilled experience in interacting and collaborating
with other physicians
- Able to establish relationships with different groups of
internal/external stakeholders to obtain input and buy in
- Strong problem-solver and business acumen.
- Highly organized and proficient in short-term and long-term
strategy.
- Excellent verbal, written and interpersonal skills and
communication skills.
- Evidence of continued growth and learning
- Provide clinical expertise and ability to support effective
execution of operational/clinical service line initiatives
- Ability to independently and accurately incorporate strategic
concepts into projects and guide others to do so.

Training:
- Graduate from an accredited medical or dental school as M.D.,
D.O.,
- Board-certified in their specialty Experience in supporting
clinical programs across departments and initiating and developing
programs
- 2 years experience in FQHC environment
- Credentialed and privileged according to Legacy's standards and
practices
- Experience in supporting clinical programs across departments
and initiating and developing programs preferred
- 2 years experience in FQHC environment preferred
- BLS (CPR & AED) Certified
