and discounts Our Occupational Therapist is a registered
professional who provides occupational therapy to patients of
varying diagnoses. Plans, implements, and evaluates patient care
plans to restore or maintain patient well-being. Evaluates
occupational roles and occupational performance of patients with
physical, psycho-social and/or cognitive impairments. Provides
interventions for deficits and elicits responses. Participates in
the coordination of care. ResponsibilitiesPerforms ongoing clinical
assessment, including OASIS assessments, at appropriate time points
to identify patient needs and determine level of functioning and
appropriateness of patient for home care.Develops and evaluates the
plan of care in partnership with the physician, patient,
representative (if any), and caregiver(s). Revises the POC based on
evaluations and ongoing assessment dataCoordinates care and
documents coordination with all physicians and other health care
clinicians involved in the care of the patient as needed and
appropriate.Evaluates the patient's occupational performance in the
areas of self-care, work, leisure, and rest. Assesses sensory-motor
neuromuscular, psycho-social, and cognitive-perceptual components
of occupational behavior. Assesses physical, social, and cultural
components of the patient's occupational environment. Assesses and
facilitates through direct therapeutic intervention the patient's
ROM, strength, coordination, need for and use of adaptive equipment
and environmental modifications, functional mobility and safety
issues as related to the patient's ADL performance needs.
Fabricates and maintains adaptive equipment and hand
splints.Provides patient caregivers and family education.Documents,
prepares, and completes clinical notes, phone orders and other
clinical record documentation on an ongoing basis and in a timely
manner.Provides treatment and services according to state and
federal regulations and professional standards.Applies concepts of
infection control and standard precautions in coordinating or
performing patient care activities to prevent contamination and
transmission of disease. Provides supervision to Certified
Occupational Therapist Assistants and Aides, when applicable, in
the performance of patient care that is consistent with the
patient's needs.Meets mandatory continuing education requirements
of the Agency and licensing board.Uses effective interpersonal
relations and communication skills.Promotes the Agency philosophy
and administrative policies.Demonstrates commitment, professional
growth, and competency.Participates in the Agency's QAPI
program.Participates in Agency-sponsored in-service training.
QualificationsEducation: Graduate of an accredited Occupational
Therapy (OT) program.Licensure: Current State License as an
Occupational TherapistCPR Certification: Requires current CPR
certificate, and the ability to maintain a current CPR
certificate.Experience: At least one year's experience as an
Occupational Therapist preferred in an acute care, rehabilitation,
or home health setting.Skills: Therapy skills as defined as
generally accepted standards of practice. Tech-savvy with the
ability to learn quickly.Transportation: Reliable transportation,
current Driver's License, and valid and current auto liability
insurance.
