Job Summary and Qualifications Seeking a Registered Nurse (RN) Patient Care Navigator to join our Coordinators team who will foster an environment of teamwork, quality and safety for our patients. The RN Patient Care Navigator's primary function is to build a relationship with patients and providers, to coordinate a plan of care for service lines as needed, including cardiology, orthopedics, and women's services/MFM. The RN Patient Care Navigator will also assist when available with prenatal education, tours of the maternity unit, and other tasks as assigned.What You Will Do In This Role:
    • You will serve as a liaison between the hospital, the patient, and health care professionals, including physicians.
    • You will participate in establishing, maintaining, and improving patient's health care goals through patient assessment and continuity of care.
    • You will identify and assist with opportunities for performance improvement activities on the unit and with other service areas.
    • You will communicate effectively with other care providers (physicians, nurses, business office, and medical records) to provide continuity of patient care and to meet the needs of the customers we serve.What Qualifications You Will Need:
      • Bachelors of Science in Nursing (BSN) from an accredited school of nursing required
      • Current RN Licensure in the State of Texas.
      • American Red Cross or American Heart Association Basic Life Support Course (BLS or BCLS) and Certification
      • Minimum of 3 years of obstetrical nursing experience required. Previous experience in program development, nurse navigator and / or patient educator in childbirth preferred.
      • Experience in leadership and customer service required.
      • RNC-OB, RNC-NIC, RNC-MNN, IBCLC or professional certification (Preferred)At The Woman's Hospital of Texas , our care philosophy is Woman First.
