extension Schedule Notes: One of three shifts: 7am to 5:30pm - 8am
to 6:30pm - 11am to 9:30pm Duties: "To promote pharmaceutical care
by managing the medication process, identifying and preventing
potential drug-related problems, and resolving actual drug-related
problems. Additionally, to perform the duties of a registered
pharmacist as defined by Texas Children's Hospital. All
responsibilities will be performed in a manner developmentally
appropriate for the ages of patients served (0-adult)." • Ensures
the appropriateness of all medication doses in reference to the
patient’s diagnosis, weight/body surface area, allergies, and drug
interactions as evidenced by lack of medication incidents. •
Accurately enters medication orders in computer as evidenced by
lack of medication incidents. • Ensures the medications the patient
receives are correct as documented by lack of dispensing errors
file. • Ensures medication orders are processed and doses are
delivered within established guidelines as evidenced by peer review
or by the lack of medication incident/customer complaint. • Follows
aseptic technique and infection control in the preparation of
sterile products. Insists technicians and other pharmacists
practice aseptic technique and infection control. Completes annual
re-certification and scores at least 90% on the aseptic
technique/infection control skills and written exam as documented
by a qualified observer. ? Ensures medications meet the highest
standards ? Participates in the education of patients, caregivers,
and the community ? Supports patient safety systems ? Checks all
available means of communication including e-mail, mailbox, and
voicemail every shift ? Ensures the smooth running of the pharmacy
and acts as a role model for technicians and other pharmacists as
evidenced by lack of complaints, medication incident reports, and
peer review ? Maintains investigational drug records according to
departmental policy ? Contribute to the medication management
process ? Conduct discharge medication review (difficult to obtain,
prescription accuracy) and counseling, ensuring that
patients/caregivers understand their medication therapies upon
discharge ? Participates in study initiation, maintenance and
closure Skills: • Ensures the appropriateness of all medication

degree or Ph.D. in Pharmacy required o RPH certification from the
state of Texas required o 2 years hospital staff pharmacist
experience (preferred) OR 5 years retail pharmacist experience at a
high volume store o 20 hours of training in compounding sterile
products accredited by the Accreditation Council for Pharmacy
Education or the American Society of Health-System Pharmacists
preferred Education: "Required- Bachelor's Degree Pharmacy Or Ph.D.
Pharm. D. degree" Required- RPH - Pharmacist State of Texas "The
pharmacist in this role shall be a licensed practitioner who holds
a valid, unrestricted license to conduct clinical activities."
