JOB CODE: (VS0214A3) RN/LPN/Dialysis LOCATION: Houston area, TX Only candidates with Dialysis experience will be considered. We are assisting a client in their search for experienced Dialysis RNs or LPNs for this exceptional opportunity! Our client provides dialysis solutions to patients in Nursing Home, Long Term Care, and Rehab environments and offers a competitive base pay with comprehensive benefits. Schedule: Monday to Friday (8 :00 a.m.-4:00 p.m.) depending on location. Some locations start earlier (6:30 a.m. - 3:00 p.m. or 7:00 a.m. - 3:30 p.m.). The Home Hemodialysis Registered Nurse is responsible for coordinating and providing the nursing care, training, education and home support needed for patients to perform dialysis safely in their homes. Works collaboratively with team members and participates in program development. This position supervises the LPN/LVN and PCT positions. DUTIES: 1. Clinical: Responsible for home dialysis program development; Collects, analyzes and records relevant patient health data accurately; Analyzes assessment data to identify patient problems; Develops a plan of care that describes interventions to attain expected outcomes; Implements and records interventions to attain expected outcomes in EMR; Evaluates patient progress toward attainment of identified goals; Adheres to approved, research-based practice standards; Directs and supervises patient care provided by LPN/LVNs and support personnel; Utilizes equipment and materials in a safe and cost effective manner; Performs and documents Point of Care (POC) testing and quality control in accordance with established laboratory policies and procedures; Functions as patient advocate and ensures care is delivered in a non-judgmental and non-discriminatory manner; Assures patients are informed consent and have signed for indicated procedures; Consistently participates as an active member of department performance improvement committee; Responsible for QAPI tracking and action plans to meet goals. Meets with the Nurse Manager monthly to drive QAPI goals; Uses current data and research in order to have the home program reflect industry trends that will improve patient outcomes: Develops relationships with colleagues at other home programs; Works collaboratively with the interdisciplinary team, other departments and outside institutions; Works collaboratively with team members to maintain inventory; Supports dialysis teaching day and assists with the planning; Provides nursing care, training, education, and home support to adult ESRD patients and significant others involved in their care; Assesses the need for home visits; Provides support for home patients and family members while maintaining professional boundaries; Provides direct patient care as needed in response to on-call issues; Collaborates with MD and staff on hospitalized home patients providing direct patient care as needed; and Provides supervision of the home dialysis program. 2. Performs similar or related duties as requested or directed REQUIREMENTS: Minimum one (1) year of Hemodialysis experience REQUIRED Valid State Nursing License required Associate's degree pr Diploma from School of Nursing; BSN a plus Current CNN or CDN required or must be obtained within one (1) year from date of hire Current BLS required Current driver's license Able to assess patient status and take appropriate action Able to function at a high level with little operational oversight Exemplary interpersonal skills - able to effectively communicate (verbal and written) with diverse backgrounds and different organizational levels Able to define problems and present solutions Able to sit and work at computer for extended periods of time Walk, sit, lift no more than 50 pounds Must be able to identify and differentiate colors Must be willing to travel to network facilities and sites as needed Enjoy a competitive base pay with comprehensive benefits.
