Registered Nurse Transplant Coordinator I - Pre Liver - Houston Method Work Shift: DAYWork Week: M - FJob SummaryPOSITION SUMMARYThe Pre-transplant Coordinator I facilitates and coordinates all aspects of the transplant referral, evaluation, waiting period, and peri-operative phases of transplantation. The coordinator interacts with all members of multi-disciplinary transplant team to ensure coordination of care for the pre-transplant patient.PATIENT AGE GROUP SERVEDSelect the applicable age group(s) of primary customers/patients served by this position. Communicates with candidate, referring physicians, other health care providers, payors, and transplant team members regarding the candidates status and evaluation outcome and maintains communication while candidate on waitlist. (EF)3. Uses therapeutic communication to establish a relationship with patients and families and communicates the plan of care. Identifies and responds to the clinical, psychosocial and economic needs of the transplant patient/family/caregiver during evaluation, waiting, and pre-operative transplant phases. (EF)4. Provides staff and community education in the areas of transplant and organ donation. Participates in department donor awareness events, educational opportunities, and community activities.5. Presents a professional image at work and all work-related events. Fosters teamwork approach in all interactions with peers and team members. Anticipates needs of other team members; proactively offers assistance.6. Contributes to meeting departmental and organ program targets for patient satisfaction measures on departmental dashboard. FINANCE 10%1. Maintains and submits accurate data for hospital-based statistics and regulatory agencies such as UNOS/UNET, CMS, and Medicaid in order to ensure proper department financial management.2. Contributes to meeting departmental and organ program financial measures on departmental dashboard. QUALITY/SAFETY 35%1. Identifies appropriate candidate upon referral and interacts with appropriate staff. Identifies requirements for candidate evaluation. (EF)2. Identifies possible co-morbidities in the transplant candidate and evaluates as indicated. Identifies absolute and relative contraindications to transplantation. Communicates all findings with transplant team. (EF)3. Demonstrates knowledge of, and compliance with, CMS Conditions of Participation and OPTN/UNOS policies and listing requirements. Provides data to OPTN/UNOS for listing statistics (Candidate Registration Form). (EF)4. Recognizes potential problems, abnormal diagnostic findings, and/or significant changes during the waiting phase, coordinating required care. (Quality/Safety) (EF)5. Exhibits ability to maintain and update candidate listing status per OPTN/UNOS regulations. (EF)6. Provides and maintains comprehensive documentation of evaluation process, candidate progress while on waitlist, and donor/recipient transplant information. (EF)7. Maintains and submits accurate data for hospital-based statistics and regulatory agencies such as UNOS/UNET, CMS, and Medicaid in order to ensure compliance with all governmental reporting requirements, and proper quality outcomes monitoring. (EF)8. Contributes to meeting departmental and organ program targets for quality and safety, on departmental dashboard.SERVICE 30%1. Coordinates with multidisciplinary team the evaluation process for potential transplant candidate, including, but not limited to (EF):a. Medical/Surgical evaluationb. Psychosocial evaluationc. Educational assessmentd. Laboratory evaluation including serologies, tissue typing, and antibodies per protocole. Radiological evaluationf. Nutritional evaluation2. Establishes mutual educational goals with patient and family/caregivers and provides education and appropriate resources regarding (EF): Living donation options (if applicable to organ) Evaluation process OPTN/UNOS regulations Organ allocation Waiting phase Transplant Process Risk/benefits, including utilization of expanded donors, high-risk donors, if applicable Post-operative phase including immunosuppressive therapy and effects Potential research protocols of the transplant program Candidate/Family/Caregiver responsibility throughout the transplant process3. Collects and evaluates medical, psychosocial, and financial data for review by transplant team in the patient selection committee. (EF)4. Ensures patient and referring physician are informed in writing of transplant listing, denial, or postponement. Informs and educates patient/family/caregiver at time of listing. (EF)5. Ensures antibody screen is obtained regularly per transplant center protocol (if applicable to organ). Maintains routine health maintenance and surveillance for malignancy during the waiting period. Identifies the need for candidate re-evaluation and initiates when appropriate. (EF)6. Participates in transplant event process according to transplant center protocol. (EF)7. Contributes to meeting departmental and organ program targets for process on departmental dashboard. GROWTH/INNOVATION 15%1. ICU experience preferred. Previous transplant acute care experience is an asset. Prior Transplant Coordinator experience not required.CERTIFICATES, LICENSES AND REGISTRATIONS REQUIREDRegistered nurse with current Texas licenseCPR CertificationSPECIAL KNOWLEDGE, SKILLS AND ABILITIES REQUIRED Skilled at managing complex patient caseload and ability to prioritize/triage based on patient status. Extremely well-organized and detail oriented, self-directed. Excellent interpersonal skills required to deal with diverse patient population.
