OVERALL RESPONSIBILITIES nnTo promote family life for children with developmental disabilities at risk of going into or currently living in an institution by assisting families to bring their children home with support or to choose other families called Support Families who are carefully selected and trained to care for children in their homes on a long-term basis. nnJob nnType -Full time, 40 hours per week with company paid medical benefits nnESSENTIAL JOB FUNCTIONS nnHelp families understand and consider family life as an alternative to institutional care. Provide families with information and linkages to community supports, services, providers and advocacy.nnnnIdentify funding, resources, supports and services to support children to remain at home or to live with a Support Family. nnDevelop relationships with provider agencies that serve children with developmental disabilities to develop their interest in and capacity to create family life for children with developmental disabilities who are living in institutions or at risk for institutional placement.nnnnDevelop and implement strategies to recruit Support Families and work with providers, agencies and families to make appropriate matches with Support Families.nnnnEnsure preparation of the supports and services necessary for the child to transition successfully from an institution to family life including pre-placement activities. nnAssist in the development and implementation of outreach and public awareness efforts nnProvide consultation or technical assistance to families, providers, agencies, community organizations, stakeholders and the general public on services for children with disabilities.nnnnWork under limited supervision of manager, with considerable latitude for use of initiative and independent judgment.nnnnAbility to travel to conduct home and facility visits and to participate in planning meetings. nnRequirements nnSoftware: Microsoft, Dropbox, and 10 Key nnEducation nnRequired: Bachelors degree in a field related to the work to be performed, such as social work or other human services.nnFour years or more experience may be substituted for degree, including personal experience caring for a family member with a disability. nnPreferred: Masters degree in related field nnExperience nnRequired: Two years experience working with people with disabilities and/or in the foster care field.nnnnPreferred: Three or more years experience coordinating a program or project, managing Medicaid programs, providing service coordination, and/or permanency planning. nnBenefits nnMedical Benefits nn401K retirement nnSalary: 40,000 to 45,000 nn
