Job Description:
JOB TITLE:Qualified Mental Health ProfessionalOVERVIEW:The Wrap
Facilitator uses the National Wrap Around Implementation Center
(NWIC) Wrap Around model to provide intensive case management
services to children and adolescents who are at risk of out-of-home
placement due to psychiatric hospitalizations, juvenile justice
involvement or CPS involvement.The Wrap Facilitator needs to be
flexible, accepting, strengths-based, team oriented, able to
communicate effectively, reliable, and able to drive treatment
based on the desired outcomes as determined by the individuals
served.Our service area for this position covers the following
counties: Gregg, Harrison, Marion, Rusk, Upshur, and
Panola.RESPONSIBILITIES:ESSENTIAL DUTIES AND RESPONSIBILITIES WILL
VARY DEPENDING ON PROGRAM AND POSITION.(May include but not limited
to the following):Provides intensive case management services.Meets
with families in their homes, the community, schools, and other
locations that may be beneficial to the family.Carries a very small
caseload due to the intensity of the services provided as well as
severity of cases.Assists families in creating crisis plans.Assist
clients in identifying and articulating their requests and needs
for services and supports.Identifies needs and strengths of the
family.Assists in the development of a team of natural and
professional supports to assist the family in achieving their
goals.Negotiate and facilitate the array of services and supports
needed to address the client's goals and desired outcomes as
identified.Provide behavioral intervention services, training and
support to assigned caseload.Ensure planned services and supports
are implemented and appropriately documented.Connect consumer with
appropriate services and supports, as neededDocument all meetings
and notes pertaining to consumer plansCOMPETENCIES:The competencies

skills.Demonstrates good clinical insight and judgment.Dedicated to
the success of children and family teams.Skilled in speaking in
front of groups.Strong organizational and time management
skills.Ability to effectively communicate verbally and in writing
in the English language.Record data neatly, coherently, accurately,
and objectively.Follow service implementation processes designed to
meet the needs of people served and funding-source
requirements.Ability to proficiently work with computers and
electronic records.QUALIFICATIONS:EDUCATION AND
EXPERIENCE:Bachelor's degree or master's degree in a Human Services
field, including but not limited to: psychology, sociology, social
work, rehabilitation, counseling.CERTIFICATIONS, LICENSES,
REGISTRATIONS:Valid Driver's LicenseTRAVEL:Local travel.Field-based
assignments require travel within the counties that we serve in
this area (Gregg, Harrison, Marion, Rusk, Upshur, and Panola.)WORK
