• Implement and conduct individual nutrition consultation and care for high risk participants using Nutrition Care Process systematic model
• Assist with staff training to meet needs and optimize Nutrition Education and counseling of participants -
• Review, approve and follow up on Medical Request Non-Contract Formulas, according to State and Local WIC Policy and Procedures - -
• Coordinates nutrition services with other departments, community services, and primary care providers, making appropriate referral and follow up -
• Summarizes, prepares and submits monthly reports as required
• Performs special projects as requestedWORKING CONDITIONSThe position is physically comfortable, the individual has discretion about walking, standing, etc.
