Anti-Violence Program The Montrose Center is seeking a skilled and
survivor-centered Case Manager to join our Anti-Violence Program.
While housing stability is a primary focus of this role, the Case
Manager also provides holistic advocacy and coordination across
multiple systems to meet survivors evolving needs. The ideal
candidate is collaborative, adaptable, and deeply committed to
trauma-informed, LGBTQ-affirming care, and will contribute
meaningfully to programs that promote safety, stability, and
self-defined independence. Key Responsibilities: Provide
trauma-informed, survivor-centered case management services to
individuals experiencing domestic violence, sexual assault, hate
crimes, and/or human trafficking. Conduct screenings and
comprehensive needs assessments for survivors and any accompanying
minor children, including assessment of housing barriers, safety
concerns, financial needs, and service goals. Develop, implement,
andmonitorindividualized service plans, which may include
housingassistance, emergency shelter placement,
financialassistance, legal advocacy, healthcare coordination,
counseling referrals, and other supportive services. Provide crisis
intervention services andparticipatein on-call rotationsin
accordance withprogram requirements. Accompanysurvivors,
asappropriate, to hospitals, law enforcement agencies, courts,
housing appointments, and other service providers to provide
support and advocacy. Inform survivors of their rights, available
resources, and service options, and support informed
decision-making without coercion. Establish andmaintaineffective
working relationships with housing providers, shelters, and
community partners, and serve as a liaison between survivors and
external systems tofacilitateaccess to services. Participate in
outreach, prevention, and education activities, including community
meetings, presentations, and events, as assigned.
Maintainaccurateandtimelycase documentation, data entry, and
reports in compliance with agency policies, HMIS requirements, and
funding source guidelines. Qualifications: Graduation from an
accredited college or university with a Bachelors degree preferred,
but relevant experience with Housing, trauma-informed, and
survivor-centered case management is priority. Masters degree in
Social Work, Psychology, Nursing, or Public Health or equivalent
degree that provides the capacity for the employee to be licensed
as an LMSW or LPC preferred. Two (2) years experience in
anti-violence prevention or service provision of which an
internship or field placement preferred. Licensed Master Social
Worker, Licensed Clinical Social Worker, or Licensed Professional
Counselor preferred. Bilingual in Spanish preferred. Equal
